Respite:
We provide 1:1 respite services for clients with various diagnoses who participate in leisure and recreation activities to work towards their tailored goals. We work as a collaborative team with the clients, caregivers, school professionals, and county social workers to develop goals surrounding their areas of need. Our Direct Service Professionals are equipped to teach social and emotional regulation, daily living skills, and coping mechanisms to manage behavioral responses to enable growth across their social systems.
Daily Living Skills:
These services are built into our respite care according to clients individual goals. Our Direct Service Professionals are equipped to promote independence with hygiene, cooking, cleaning, and personalized skills. We provide occasional weekend sessions to focus on specialized topics for small groups. Our small groups are staffed 3:1, allowing adequate attention to each individual while they work on building practical hobbies and healthy routines.
Discovery/Career Planning:
These services are built into our respite care according to clients individual goals. Our Direct Service Professionals are equipped to help research careers, consider realistic future plans, learn to budget, and are excellent models of a strong work ethic. These services may also be offered as weekend sessions to focus on specialized topics for small groups. Our small groups are staffed 3:1, allowing adequate attention to each individual while they work on building employable skills.
**Future expansion plans include partnering with local companies to provide opportunities for employment while clients receive on the job coaching from our Direct Service Professionals.**
Health & Wellness:
These services are built into our respite care according to clients individual goals. Our Direct Service Professionals are equipped to teach daily routines, self care, healthy eating, and physical/mental health. These services may also be offered as weekend sessions to focus on specialized topics for small groups. Our small groups are staffed 3:1, allowing adequate attention to each individual as they learn about healthy activities, young women's health, upcycling, gardening, and more!
Transitional Services:
We know that aging out of the Children's Long Term Support program can be a difficult transition for many clients. We want to help bridge the gap in support to the best of our ability. Around the time of our clients 16th birthday we will begin discussing goals for after they age out of CLTS and creating a plan for the transition to adult services. We will collaborate with organizations to ensure smooth transitions into adulthood and so strive to support clients long-term.
