Submit A Referral

Department of Human Services Social Workers located in Winnebago County WI, can submit a referral for a client through this form. 

Currently we do not accept private pay services. Potential clients must have a social worker and be enrolled in the Children's Long Term Support Program in Winnebago County. Please see our resources page for a link to apply for a CLTS worker.
